Email form info to client

Hey guys

Need some help here pls. I have spoken to the service provider where the following page is hosted: http://www.salvage-sa.co.za/question/question.html

I need the form to be posted to a client of mine. At the moment i need to test it first so its going to rc51sp@yahoo.com my personal email address. The service provider is using jmail functionality, and i have never used it.

So what i need please is first off if my form page is correct and the check boxes and text boxess info will be captured to the supplied .txt file and then that it will be mailed to whatever email address i supply in a .txt or .xml format.

Please guys help here
